ONLINE PRESCRIPTION RENEWAL
You have requested access to our online prescription renewal system.
This system is available only to patients of our Practice.
If you are a new user, you will need to create a user name
and password to gain access to our secure server. You can
request refills for yourself and/or family members using your
user name and password that you will create during the signup process. A separate
user name for each family member is not required.
Please note the following conditions of use for our online prescription
renewal service:
- Use of this service is limited to requesting refills of
existing medications which have been prescribed by a provider
from our Practice. Please do not use the service to request
a medication change or to request a prescription to treat a
new medical problem.
Information you provide using this service will be submitted
via a secure e-mail to our office.
- You may request a refill for up to 4 medications at each
log-on to the system.
- To process your request, your medical record will be reviewed
to verify the prescription information. If you have not had
a recent visit to our office, we may request that you make an
appointment.
We may need to contact you to discuss your renewal. If we need
to contact you, we will contact you by telephone. We do not
use e-mail as a means of patient contact.
Regardless of the time of your submission to us, you should
expect that your request will be processed by the end of
the next business day, excluding Saturdays, Sundays and holidays.
If you are out of medication and can not wait until that time,
please call the office (8:30 AM – 5:00 PM Mon. – Fri.) After
hours, an
on-call provider is available in an emergency to provide
medication refills.
- Please be aware that we will not fax or mail prescriptions
to mail-order pharmacies. Mail order prescriptions will be mailed
to your home address.
- You will receive an auto-reply e-mail acknowledging
receipt of your refill request. This is the only
acknowledgement you will receive. If you do not receive an
acknowledgement, please contact the office to request your
refill.
